Licensed Independent Social Worker (LISW) – Cognitive Care Program at Victoria House

Location: Austintown, OH

Employment Type: Part-Time (with potential for expansion)

Compensation: $45/hour + performance-based bonuses

Reports To: Program Director

About the Role

We are seeking a Licensed Independent Social Worker (LISW) to play a key role in our Cognitive Care Program at Victoria House, an assisted living facility dedicated to improving cognitive health in seniors. This program integrates neurofeedback, mental health counseling, and evidence-based interventions to support residents experiencing cognitive decline, dementia, or other neurological conditions. The LISW will lead patient assessments, provide therapy, collaborate with healthcare professionals, and ensure compliance with Medicare and insurance guidelines while working to enhance the well-being of our residents and their families. This is an exciting opportunity to be at the forefront of an innovative care model that seeks to redefine cognitive health support in assisted living settings.

Key Responsibilities

  • Clinical Care & Therapy
  • Conduct comprehensive psychosocial assessments for residents experiencing cognitive challenges.
  • Provide individual and group therapy using evidence-based techniques to enhance cognitive function and emotional well-being.
  • Develop and implement personalized care plans in collaboration with medical professionals and caregivers.
  • Offer family counseling and education to help loved ones better understand and support cognitive health needs.

Program Implementation & Collaboration

  • Work closely with the neurofeedback team, physicians, nurses, and other care providers to integrate mental health and cognitive care strategies.
  • Participate in interdisciplinary meetings to discuss resident progress and adjust treatment plans as needed.
  • Educate staff on best practices for mental health and cognitive support in an assisted living setting.

Compliance & Documentation

  • Maintain accurate, timely documentation for all patient interactions, ensuring compliance with Medicare and insurance regulations. –
  • Assist in developing standard operating procedures (SOPs) and protocols for the Cognitive Care Program.
  • Stay up-to-date on state and federal mental health regulations to ensure ethical and legal compliance.

Resident & Family Engagement

  • Provide support groups and workshops for residents and their families to improve coping strategies.
  • Advocate for resident needs and serve as a liaison between healthcare providers, families, and facility staff.
  • Promote a positive, engaging environment that encourages cognitive stimulation and emotional support.

Qualifications & Experience

  • Master’s Degree in Social Work (MSW) from an accredited program.
  • Active LISW license in Ohio (or ability to obtain).
  • Minimum 2 years of clinical experience, preferably in geriatric, cognitive health, or neuropsychology settings.
  • Familiarity with neurofeedback, cognitive behavioral therapy (CBT), and other relevant therapeutic modalities is a plus.
  • Strong knowledge of Medicare/Medicaid billing practices for behavioral health services.
  • Ability to work collaboratively in an interdisciplinary team while maintaining a high standard of care.
  • Excellent communication, documentation, and problem-solving skills
